Build a form, publish it, assign it to a patient, and follow their submission.
How do I build a form and assign it to a patient?
Who can do this: clinicians, and Full Access or Administrative Staff / All Billing members. These roles have Forms in the sidebar. Clinicians can assign forms only to patients whose care team they're on, with Access Charts on.
Build a form
- Open Forms, then select Create Form.
- Under Form Details, enter the Form Name and an optional Description, and choose a Category. Tick Requires patient signature if the patient must sign.
- Under Add Field, add each question: Short Text, Long Text, Number, Email, Phone, Date, Dropdown, Multi-Select, Checkbox, Radio Buttons, File Upload or Section Header.
- Give every field a label. Tick Required for questions the patient must answer. For dropdowns and choices, fill in each option.
- Select Save & Publish to make the form ready to assign, or Save Draft to keep working on it later.
Only a published form can be assigned to a patient. There's no way to publish a draft from the Forms page yet, so select Save & Publish when the form is ready.
Assign a form
- Open Forms. On the Templates tab, find the form and select Assign.
- In Assign Form, choose the Patient.
- Select Assign form.
The form is added to the patient's record as Pending, and they see a "form to complete" reminder on their dashboard.
The patient isn't emailed, texted or sent a link. Patients can't open or fill in assigned forms in the app yet, so let them know directly and collect the answers another way.
Follow a submission
The Submissions tab lists assigned forms. Each row shows its status, such as pending, in progress or completed, and when it was assigned or completed. From a row you can:
- View the patient's answers.
- Download a copy.
Use Preview on the Templates tab to see a form the way the patient will.
If something goes wrong
- "Please enter a form name", "Please add at least one field" or "All fields must have labels" means the form isn't complete. Fix it and save again.
- "A form with this name already exists. Please choose a different name." means your clinic already has a form with that name.
- A message saying the form "was saved as a draft but could not be published" means the form exists but can't be assigned. Contact GRAICE support.
- Access Denied with "Only clinicians, coaches, and administrators can create forms." means your role can't build forms.
- "Failed to load form submissions. Please try again." means the list didn't load. Refresh the page.
