How do I find a patient and open their chart?

Search the Patients list, open a chart, and learn why the clinical chart can be hidden from you.

How do I find a patient and open their chart?

Who can do this: clinicians and coaches. Admins see the patient roster but not clinical charts unless they hold chart access.

    1. Select Patients in the sidebar.
    2. Type in Search by name or ID.... Narrow the list with the Gender and Portal access filters, or select Advanced for Age Range.
    3. If you see My Patients and All Patients tabs, use them to switch between your own roster and the whole clinic.
    4. Select a patient. The chart opens on Profile.

Inside a chart, the left menu switches to that patient's sections, such as Profile, Notes, Messages, Prescriptions and Referrals. Select All Patients at the top of that menu to go back to the list. Press ⌘K to jump to another patient.

Why is the clinical chart hidden?

Chart access is per patient. You see the clinical chart only when you are on the patient's care team.

If you are not, the page shows Demographics view with the message "You're not on this patient's care team, so the clinical chart is hidden." Select Assign a provider to put a provider on the care team, if your role allows it. Otherwise ask your clinic admin.

    If something goes wrong

    • Patient Not Found means the patient does not exist or you do not have access. GRAICE shows the same message for both on purpose.
    • Unable to load patient data means a temporary error. Select Try Again.
