How do I create and track a referral?

Create a referral from the patient chart, then approve, route, schedule and complete it from the Referrals page.

How do I create and track a referral?

Who can do this: clinicians and coaches. Coaches request referrals and a clinician approves them.

    Create a referral

    1. Open the patient's chart on Profile.
    2. In the Referrals section, select the + (New referral) button.
    3. Choose a Specialty and Priority. Optionally pick a Receiving Provider from your clinic.
    4. Enter a Reason for Referral of at least 10 characters. Add Clinical History, Diagnosis / ICD-10 and Questions for Specialist if useful.
    5. Select Create Referral. Coaches see Request Referral.

    Track it

Select Referrals in the Tools group. Use the tabs All, Pending Review, Draft, Sent, Accepted, Scheduled and Completed. Select a referral to open its details.

    Clinicians can then:

    1. Select Approve Referral on a coach's request.
    2. Select Route to Receiving Provider, then Route Referral, to hand it to a provider in your clinic. This button needs a receiving provider on the referral.
    3. The receiving provider selects Accept Referral, or declines.
    4. Select Schedule, pick a date and select Schedule again. Later, select Mark Completed.

Use Cancel Referral to stop one.

GRAICE does not send referrals outside your clinic

Routing is an in-app handoff only. If the specialist is outside your clinic, send the referral yourself by fax, phone or your own secure email, then record the outcome in GRAICE.
