How do I use AI Scribe to draft a SOAP note?
AI Scribe listens to the visit and drafts a SOAP note. The draft is never final. You review, edit and sign it.
Who can do this: clinicians on the patient's care team. Otherwise Start Recording is disabled and the panel says "Recording unavailable for this patient."
- In an open visit, select the Chart tab, or select AI Scribe at the top of the visit.
- Select Start Recording.
- In Confirm Recording Consent, tell the patient what is recorded and how it is protected. Select Patient Consented — Start Recording. Consent applies to today only, so if the patient already consented today, recording starts without asking again. Select Decline to stop.
- Allow microphone access if your browser asks.
- Use Pause, Resume and Stop as needed. Select Stop when the visit ends.
- GRAICE drafts the note. Edit each section, then select Save Draft or Sign Note.
Audio is streamed for transcription and is not stored afterward. The transcript and note are stored encrypted in the record.
No microphone?
Select Type Transcript, paste or type the transcript, then select Generate SOAP Note.
If something goes wrong
- "Microphone access denied" means you must allow the microphone in your browser settings.
- "Transcript may be incomplete" means part of the visit is missing. Check the note closely before you sign.
- "Please confirm recording consent with the patient again" means the consent on file is missing or out of date. The consent dialog opens again. Repeat step 3.
