How do I find and sign my unsigned notes?

Use the Notes page to clear your Awaiting Signature queue and browse notes across your patients.

How do I find and sign my unsigned notes?

Who can do this: clinicians sign notes. Coaches can open Notes and read notes but cannot sign them. Other roles see "Clinical notes are only available to providers."

    1. Select Notes in the sidebar. The page opens on Awaiting Signature, a queue of your own notes still waiting for your signature. A badge shows how many.
    2. Select a note to read it.
    3. Check the note, then select Sign Note. GRAICE confirms with "Note signed."

Only the note's author sees Sign Note, and only on a draft. You can read a colleague's note but not sign it.

    Browse other notes

Select All Notes to see notes across your patients. Search by patient, provider or content. Filter by status (Draft, Pending Review, Signed, Amended) and by note type.

    Signed notes are final

A signed note cannot be edited. To add to it, use the visit's note editor right after you sign there: select Add Addendum, type your text, then select Sign & Add Addendum. The Notes page has no addendum button.

    Where else to sign

You can also sign inside a visit from the Chart tab. Complete Visit will not finish while the note you started in that visit is unsigned. See Run a visit.

    If something goes wrong

    • "Failed to sign note" means the note was not signed and is still a draft. Try again.
    • "Unable to load clinical notes" means a temporary error. Select Retry.
