How do I import a C-CDA record?
Upload a C-CDA file from another EMR, match it to a patient, and approve or reject each imported record.
How do I import a C-CDA record?
Who can do this: clinicians and coaches. The import page redirects other roles to Home.
- Select Patients, then select CCDA (Import C-CDA).
- Drag a file onto the page or select Browse Files. Files must end in
.xml,.cdaor.ccdaand be under 10MB. - Select Upload & Parse. The steps run Upload, Match Patient, Review, Complete.
- If GRAICE cannot match the file to one patient, pick the right patient on Match Patient and select Confirm & Import. If no one matches, the page says "No matching patients found in your organization." Create the patient first, then import again.
- On Import Complete, select Review Records.
- On Review Imported Records, filter by record type, then use Approve or Reject on records. You can select several records, or use Approve All.
Records wait under Pending Review until you approve or reject them.
If something goes wrong
- "Please upload an XML file (.xml, .cda, or .ccda)" or "File must be under 10MB" means the file failed the check. Choose another file.
- If your account lacks chart access, the upload succeeds but you see "Ask a clinician with chart access to upload it."
