Sign papers your clinic sent you, upload insurance cards, and download your documents.
How do I view, upload and sign documents?
Open My Health Records, then My Documents. Your documents, papers to sign and signed agreements are all here.
Sign a document your clinic sent
When your clinic sends paperwork, a box at the top says 1 document needs your signature (or how many).
- Click Review & sign.
- Read the document.
- Tick I have read and acknowledge this document.
- Click Acknowledge.
Click Back to leave without signing. Signed items move to Already acknowledged.
Upload a document
- Click Upload Document.
- Choose the file and enter a Title.
- Choose a Category: Insurance or Other.
- Add a description if you like, then click Upload.

Find and download
Search by name or filter by Category. Click the eye icon to see details. Click the download icon to save the file.
Agreements you signed
Signed agreements lists what you accepted at signup, such as Terms of Service and Notice of Privacy Practices. Click Certificate, where shown, to save a PDF.
If something goes wrong
- Please select a file means you didn't pick one.
- Could not acknowledge: try again. If it keeps failing, message your care team.
